Office Policies

Health History

Before your first session you will be asked to fill out a Client Intake Form.

To provide proper therapy at every subsequent session it is the client’s responsibility to always inform the therapist of new and existing health conditions.

Confirmations

Appointments will be confirmed the day before the scheduled massage.

Cancellations

Cancellations are sometimes unavoidable. Please call within 24 hours notice to allow ample time for filling that appointment and to avoid a fee.

Tardiness

Please arrive to your session at your scheduled appointment time.

If you are late for your massage appointment you will be provided as much time as available within the scheduled time slot. You will, however, still be charged the full amount for your scheduled session.

Payment

Payment is due upon receipt of services. Cash, check or credit card  are accepted. Please make checks payable to Dana Errico.

Sexual Misconduct

Sexual misconduct is forbidden. Client understands that any illicit or sexually aggressive remarks, advances or gestures will result in the immediate termination of the session and will be liable for full payment of the scheduled appointment.

Physician Referral

A physician referral is generally not required in order to receive massage therapy. After speaking with your therapist and reviewing health history information a determination will be made prior to your session if a doctor’s note is required.

*Massage therapy is not a substitute for medical examinations and/or diagnosis. It is recommended that you see a physician for any physical ailment you have.
